| 1. Please prepare the following items before sending the case or digital case to us: |

a. Fill in an ordersheet, write to us as much detailed information as possible; such as dentist name, patient name, tooth number, service items needed, single crowns or bridge, dentine color, color required, etc. |
| b. Silicone working impression (model), opposite impression (model) and study impression (model) if necessary |
| c. Bite registration and picture, if necessary. |
| d. Please include all implant parts if the case is an implant, include also lab parts. |

e. Prepare airway bill for shipping (you can choose whichever shipping courier available at your place, but the following is quickier, such as DHL, UPS or Fedex). You may also need to prepare a commercial invoice to show the Customs and a non wooden declaration paper. Please contact our service person, we have those format you can use. |
