How to ship cases to us |
| 1. Please prepare the following things before sending your case or even a digital case: |

a. Write an ordersheet, fill with as much detailed information as possible; such as names, tooth number, shade, service items needed, single crowns or bridge, return dates, designs, special comments etc. |
| b. Prepare working impression (model) and opposite impression (model), study impression (model) if necessary. |
| c. Bite registration or bite block (if necessary) |
| d. Please include all implant parts if the case is an implant, including lab parts |

e. Prepare airway bill for shipping (you can choose whichever shipping courier such as DHL, UPS or Fedex), You may also need to prepare a commercial invoice for Customs and a non wooden declaration paper. Please contact our service person, we have those format you can use. |
