| 1. Please prepare the following items before sending your case or a digital case: |

a. Write an ordersheet, fill it with as much detailed information as possible; such as dentist name, patient name, tooth to be bleached, etc. |
| b. Prepare working impression (model) and opposite impression (model) |
| c. Prepare bite registration (if necessary) |

e. Prepare airway bill for shipping (you may choose whichever shipping courier such as DHL, UPS or Fedex. You may also need to prepare a commercial invoice and a non wooden declaration paper. Please contact our service person, we have those format you can use. |
