1. Write an ordersheet, fill it with as much information as possible; example, names, tooth number, service items needed, return dates, designs, color, special comments etc. |
| 2. Please send us working impression (model) and opposite impression (model), study impression (model) if necessary |
| 3. Please include all implant parts if the case is an implant, including lab parts |

4. Prepare airway bill for shipping (you can choose whichever shipping couriers, or the following, such as DHL, UPS or Fedex), You may also need to prepare two copies of commercial invoice and a non wooden declaration paper. Please contact our service person, we have those format you may use. |
